Purpose

Allentown School District seeks to ensure that all eligible children within the City are enrolled. Students are encouraged to learn and parents are invited to participate in the education process. Enrollment packets are available at any of the following locations.

Description of Services

Parents or court-appointed guardians may enroll a student new to the District. Students residing with an adult other than the natural parent are enrolled following receipt of a Form 1302, required by Pennsylvania law, in addition to all general enrollment requirements as follows:

Age: Kindergarten - child must be 5 years old on or before midnight, September 30.

Age: First Grade - child must be 6 years old on or before midnight, September 30.

Verification of Birth Date by any of the following: original birth certificate, baptismal certificate, hospital birth record, passport, adoption papers, green card, notarized affidavit of parent/guardian.

Proof of Residency A utility bill or service agreement for gas/oil, electric, phone, cable TV, a signed current property lease, or a notarized statement of residence from the parent/guardian. (Homeless students are not required to prove residency for school enrollment.)

Student Health Information Kindergarten and First Grade – proof of physical exam within the past year or consent for school physical.

Proof of Immunization Record of child’s immunization with dates.

School Records Transfer Card from last school attended.

Helpful Records An academic transcript or report card, the student’s social security number and, for special education students, an IEP and copies of assessments.
