Employment Requirements / Clearances

REQUIRED FORMS PRIOR TO EMPLOYMENT:

In accordance with PA Department of Human Services guidelines, all prospective employees must be able to provide following documents before starting employment in the District. Once employed, all employees are required to submit updated copies of each document every five (5) years:

PA State Criminal Record Check (Section 1-111 of the Pennsylvania School Code – Act 34) - This form can be accessed from the PA State Police website at https://epatch.state.pa.us or download here. COST: $22.00

Child Abuse Clearance (Sections 6354-6358 of the Public Welfare Code – Act 151)This form can be accessed from the PA Department of Human Services website at https://www.compass.state.pa.us/cwis/public/home. COST: $13.00

Fingerprint Clearance - The registration website is available online 24 hours/day, seven days per week at https://uenroll.identogo.com. Service Code: 1KG6XN

Please register first at the website above before arriving to get fingerprinted.
You will need to bring your receipt with the UEID number to the fingerprinting location as well as an acceptable form of id. Acceptable forms of id are listed on the website.

Sexual Misconduct/Abuse Disclosure Release – This information and formapplies to prospective school entity employees and independent contractors who work with them. All applicants who will have direct contact with children must complete this form. It must be completed by the following organizations:

(1) The applicant’s current employer (regardless of whether the employer was a school entity and/or where the applicant had direct contact with children);
(2) All former employers of the applicant that were school entities; and
(3) All former employers where the applicant was employed in a position where he or she had direct contact with children.

Separate Sexual Misconduct/Abuse Disclosure Release forms must be submitted for each previous employer that falls within the categories above. Under Act 168, the hiring entity
