In accordance with PA Department of Human Services guidelines, all prospective employees must be able to provide the following documents before starting employment in the District. Once employed, all employees are required to submit updated copies of each document every five (5) years:

1.  PA State Criminal Record Check  - This form can be accessed from the PA State Police website.
2. PA Child Abuse Clearance – This form can be accessed from the PA Department of Human Services website.  
3. Federal Criminal History Record (Act 114) Fingerprint Clearance - The registration website is available online 24 hours/day, seven days per week at https://uenroll.identogo.com. Service Code: 1KG6XN

Please register first at the website above before arriving to get fingerprinted.
You will need to bring your receipt with the UEID number to the fingerprinting location as well as an acceptable form of id. Acceptable forms of id are listed on the website. After you are fingerprinted, please provide the HR Department with a copy of your receipt providing the UEID number.

Sexual Misconduct/Abuse Disclosure Release (Act 168) – This information and form applies to prospective school entity employees and independent contractors who work with them. All applicants who will have direct contact with children must complete this form. It must be completed by the following organizations:

(1) The applicant’s current employer (regardless of whether the employer was a school entity and/or where the applicant had direct contact with children);
(2) All former employers of the applicant that were school entities; and
(3) All former employers where the applicant was employed in a position where he or she had direct contact with children.

Separate Sexual Misconduct/Abuse Disclosure Release forms must be submitted for each previous employer that falls within the categories above. Under Act 168, the hiring entity

In accordance with Act 24 of 2011 and 82 of 2012, all school employees are required to report to the Superintendent or the Director of Human Resources within 72 hours of any arrest or conviction of a Reportable Offense listed in Section 111(e).  The Arrest/Conviction Report Form shall be used for these reports. The law provides that willful failure to timely report any such arrest or conviction can result in termination of employment.

The Guest Teacher Program is a service the Carbon Lehigh Intermediate Unit #21 offers to the community and our local school districts. Individuals who have obtained a bachelor's degree from an accredited college may apply to participate in the program. The program's purpose is to provide quality substitute training to individuals with limited or no teaching experience.

The program is conducted in order to provide participating Districts and Career and Technical Institutes with trained substitute teachers. 

Each Guest Teacher exits the program with the basic understanding to be a substitute teacher on a day-to-day basis and an emergency permit issued by the PA Department of Education which makes the Guest Teacher eligible to substitute teach in any participating District and Career and Technical Institutes. The opportunities are rewarding and gratifying.

If the Guest Teacher wishes to be considered for hire by the district the district only needs to provide hiring information to the candidate. Prior to the Meet and Greet, the Guest Teachers will receive the following packet which include copies of the employment application, resume, reference letters, transcripts, clearances, PA school health form, employment verification, and immigration form (I-9).

To participate in the program the billing is structured based on the number of districts participating in the Summer Guest Teacher Program. There is a $30 fee charged to the Guest Teacher which will be used to offset the district fee.
